Our social services department focuses on the psycho social well being of each resident. They serve as the patient advocate and to help improve the quality of life for each resident. The social worker helps coordinate appointments and transportation with outside physicians and specialists as well as refer to community resources to help each resident have as much independence as possible.
Care Transitions/Discharge Planning
Our goal is to help each resident transition back to their home or preferred living environment. This takes an interdisciplinary approach from the medical/nursing, rehab, and social services teams in order to make each transition safe and sustainable. Our social worker serves as the primary discharge planner and works with each resident to coordinate their transition home. This involves working with the rehab staff and resident/caregivers to make sure they are properly trained for success, and coordinating with home health agencies and other community resources. We strive to make sure that each discharge is properly coordinated so it’s more like a transition to a lower level of care at home.
Sherwood’s activities program is designed to help provide an uplifting and positive environment for each resident. Activities are tailored to match the functional abilities of our residents and to keep them physically and mentally engaged.
